The Map of Medicine lists care pathways for a wide range of conditions and diseases. You can get information about diagnosis and assessment, treatment and referrals and on-going care. It includes primary care pathways as well as acute care pathways. You can also see the reference list that has been used to develop the care plan.

The database also includes pathways from our local NHS organisations as well as general pathways and pathways from other NHS organisations. You will need your Athens username and password to access the Map of Medicine with all its full features. There is a freely available version, though you are still required to register.
