Health Library forms offline

 Unfortunately this week's IT issues have affected the underlying system we use to deliver our online forms. This service has been taken offline temporarily while essential maintenance and upgrade work is carried out.

Health Library forms will therefore be unavailable during this period, meaning our users will not be able to submit requests online. We are aiming to have the service back online early next week and will provide a further update once the work has been completed and the service is available again.
